At Animal Health Hospital we want to make your veterinary experience as simple and easy as possible. From patient and client forms emailed directly to us, to online post-operative instructions, and emergency vet services that will help you find veterinary services when we are unavailable.

  • Emergency Services

    We can and would like to assist you with emergencies that happen during our business hours. We are open:

    - Monday-Friday 7:00 am – 5:30 pm
    - Saturday 7:30 am – 12:00 noon

    Please call us before or as you’re heading to our office so that we can be prepared as soon as you arrive, 520.885.2364.

    For Emergencies that happen at any other time please see the resources listed on the Emergency page or click the button below to find the nearest veterinary emergency hospital.

  • Post-Operative Care

    Post-operative care provided at home is important to the final outcome of your pet’s surgery. While necessary post-operative care is specific to each surgery, there are some instructions that are common to all surgeries and dental procedures. Please call 520-885-2364 for specific instructions.

  • Patient Forms

    Client and first pet information
    New clients should fill out this form. This form gives Animal Health Hospital the necessary information to generate a record for you and your pet.

    Additional pet form
    Clients should fill out this for each new pet that will be visiting Animal Health Hospital.

    Emergency Treatment Authorization
    Clients should fill out this form to authorize treatment when you are away on vacation or on business.
